By attaching my signature, I acknowledge and I agree that participating or observing in the activities sponsored and offered by CBDOJO INDOOR RECREATIONAL SERVICES has inherent risks, including but not limited to grappling, takedowns, climbing/playing with aerial apparatus, dance and other fitness activities. Those risks include but not limited to:

1. Injuries from rolling or falling from being in contact with people or objects in the dojo
2. Injuries resulting from existing physical pains or disabilities
3. Injuries resulting from negligence or inadequate training
4. Injuries that may cause death for negligent misjudgment or misuse of any object in the dojo
5. Loss or damage to property (Please do not bring valuables and keep an eye to your personal things)

I am aware of these and other inherent risks that come with my or my child’s participation or observance of such activities in the dojo. I assume COMPLETE responsibility and liability for these risks and for any injuries or inconvenience that may result from my or my child’s participation or observance, even if injuries occur in a manner that is not foreseeable at the time I sign this agreement. I will solely be responsible for any loss or damage I or my child sustain including personal injuries, loss or damage to my property, or damage arising from death. This document is legally binding and by signing it, I am giving up my rights to compensation through the courts.
